Tacrolimus-based immunosuppression in pediatric renal transplantation.

چکیده
TACROLIMUS has been shown to be a safe and effective immunosuppressive agent in adult renal transplantation with significant advantages, including the ability to taper corticosteroids in up to 65% of recipients. 1-4 The use of this agent in the pediatric population is less well described. We and others have previously reported that its use in pediatric patients is associated with excellent patient and graft survival results in a relatively small number of patients.59 Here we report our expanded experience on the use of tacrolimus in pediatric renal transplantation.

Role of Immunosuppression Minimization Protocols in Renal Transplantation
The subject of transplant immunosuppression has generated significant interest in recent years. Excellent immunosuppression, advances in surgical technique, post-transplantation care, and infection control have resulted in excellent outcomes. There is widespread support for the notion that the fundamental objective in transplant immunology should be the achievement of specific graft toleran...
متن کاملTacrolimus and diabetic ketoacidosis after kidney transplantation in a 15-year-old girl (Case Report)
Tacrolimus is a main drug for induction and maintenance immunosuppression for patients with kidney transplants in many centers. One of important side effect of drug is post-transplant diabetes mellitus. Of course, diabetes ketoacidosis (DKA) is rare. In this report, a 12-year-old girl with DKA was presented after 45 days of kidney transplantation.

Between March 27, 1989 and December 31, 1997, 1316 kidney transplantations alone were performed under tacrolimus-based immunosuppression at our center. Posttransplant lymphoproliferative disorders (PTLD) developed in 25 (1.9%) cases; the incidence in adults was 1.20/c (1511217). whereas in pediatric patients it was 10.IO/c (10/99; P<.OOOl). PTLD was diagnosed 21.0±22.5 months after transplantat...
متن کاملConversion from tacrolimus/mycophenolic acid to tacrolimus/leflunomide to treat cutaneous warts in a series of four pediatric renal allograft recipients.
BACKGROUND The challenge of immunosuppression in pediatric renal transplantation is to balance preventing rejection while avoiding infectious complications. A dermatological complication of immunosuppression is viral warts, which cause significant disfigurement and increase the risk of skin malignancy. METHODS We present three pediatric and adolescent renal allograft recipients with multiple,...
متن کاملWithdrawal of Immunosuppression in Pediatric Liver Transplant Recipients in Korea
PURPOSE We identified pediatric liver transplant recipients with successful withdrawal of immunosuppression who developed tolerance in Korea. MATERIALS AND METHODS Among 105 pediatric patients who received liver transplantation and were treated with tacrolimus-based immunosuppressive regimens, we selected five (4.8%) patients who had very low tacrolimus trough levels. Four of them were noncom...
